<p>I know, I know, I&#8217;ve been a bit slow with the updates lately. I have no real excuse other than a serious bout of the flu. I&#8217;m in the southern hemisphere (i.e it&#8217;s summer here) and I have had both the swine flu and seasonal flu vaccinations (fortunately we have a large stockpile of swine flu vaccination here, it&#8217;s available free and on demand) but somehow I still managed to succumb to a 2 week bout of illness. Yuck.  I managed to keep my aligners in for most of it, although there were a couple of days where I simply couldn&#8217;t function without a lozenge in my mouth as my throat was so raw and painful to swallow; so on those days I&#8217;m afraid I was a little remiss.<br />
Fortunately I&#8217;m now back on form and back on track with my invisalign, so I promise I&#8217;ll try and update more regularly.  </p>
<p>As you can see from the pictures there are no massive revelations at this point in my treatment, but things are slowly moving into place. I am now on 3/9 of my upper refinements and on 23/34 of my lower aligners. </p>
<p>As you can see from the first picture the movement of my crown has now caused a very ugly black line along my gumline next to the crown. Fortunately my aligners actually mask this to a great extent so it isn&#8217;t nearly so noticeable when they are in, but it is, nonetheless, a bit <em>ugly</em>. The profile view of that crown is improving (see third picture) but still isn&#8217;t nearly perfect as the front tooth still needs to move back into place a little more. I have six aligners to go, so we&#8217;ll see what happens. I&#8217;m still really looking forward to getting the crown replaced (although already having nightmares about the actual procedure!) I know that the cosmetic dentist also said that he needs to raise my gumline above that tooth, which I&#8217;m trying not to even think about at this point for fear of never going back to another dental appointment again. If you have had a gum raising procedure and can offer any kind words of encouragement, please, please do. Frankly, I&#8217;m terrified!</p>
<p>My upper refinements as mentioned in a <a href="http://myinvisalignblog.com/2009/09/invisalign-refinementsfreedom-for-teeth.html">previous post</a> are focusing on two areas, my front right tooth that needs rotating and the molar that is too long and not in position on the left side (right side in these pictures).  I was told that my nine refinements are really six aligners and three over-corrections; if my teeth look done after the first six trays  we can stop. Something I have been wondering about is, what happens if one area looks perfect after six aligners and the other doesn&#8217;t? Do I have to over-correct one area to get a finished result on the other? I&#8217;ll ask my orthodontist at the next appointment and let you know. </p>
<p>My bottom arch isn&#8217;t looking too different from straight on but I am happy to report that a real gap is opening up inside my mouth for my stray tooth to move into (see my <a href="http://myinvisalignblog.com/2009/01/invisalign-clincheck-videos.html">clincheck</a>). I can also see that the bottom arch looks less &#8220;witchy&#8221; when viewed from the front. One of the things that I disliked so much about my teeth pre treatment is that the the bottom row were both very visible when I talked and also very pointed and crooked, giving me a look that could easily scare small children! (see my sidebar for the before pictures) This far into the treatment I am happy to report that they are becoming much more uniform and whilst not straight, will no longer qualify me for a walk on part in The Wizard of Oz.<br />

<p>Apparently, in refinement they usually give <span id="SPELLING_ERROR_1" class="blsp-spelling-error">over-corrections</span>. They appear to have concluded that if something  didn&#8217;t work the first time it may need a little something extra in the refinement phase. I&#8217;m hoping if I am superlatively compliant that little extra will be unnecessary and I&#8217;ll be able to finish after 6 aligners. We&#8217;ll see.<br />
I&#8217;m a little disappointed that there are nine, but frankly, at this point I would rather get it done right, than done quickly. I still have to wear my bottom aligners anyway as I am still in the middle of the first run of those so it really doesn&#8217;t make a huge amount of difference. The only real problem is that the upper aligners are so much more visible, and that you can feel them in your mouth more too.</p>
<div>One thing that bothers me is that I still have my original attachments on my upper teeth. I am sure my orthodontist has kept them for a reason but I know that other people have had theirs removed before refinement impressions were taken. This sometimes means that if there are no attachments needed during refinement they don&#8217;t have any. I only wish I had remembered to query it before my impressions were taken. Anyway, it&#8217;s too late now! When I see my orthodontist in a few weeks time I will ask him why we kept them and let you know. I suppose one consolation is that the attachments do keep the aligners much more stable on my teeth, they feel much tighter than they did before I had them. Perhaps that will help them to move more quickly. I live in hope.</p>

<div>One good thing is that my aligner numbers have synchronised. I will wear 21 lower whilst I wear #1 upper. This should help me to remember where I am up to. Of course, my orthodontist claims to have designed it that way. And I believe him. Really!</div>

Digitally printed on the front of the <span id="SPELLING_ERROR_1" class="blsp-spelling-corrected">CD</span> were the words &#8220;Ugly photos&#8221; along with a picture of a goat in braces, and never has a title been more apt.Whilst it was kind of him to give them to me, (I think!), I&#8217;m not at all sure that it is kind of me to put them on here, but I will, with the proviso that if you have just eaten, please, <strong><em>please,</em><span style="font-weight: normal;"> don&#8217;t look at them. They are alarmingly, distastefully, graphic and ugly.</span></strong></p>

<div>As well as giving me the photos, we spent a large part of my appointment talking about <span id="SPELLING_ERROR_2" class="blsp-spelling-error">retainment</span> and refinements, things that seemed so distant in the <a rel="nofollow" target="_blank" rel="nofollow" href="http://www.myinvisalignblog.com/2008/11/in-beginning.html">early days</a>, but now look so <span id="SPELLING_ERROR_3" class="blsp-spelling-corrected">promisingly</span> close. I&#8217;m excited even to be thinking about such things let alone talking about them.</div>
<div>After the next couple of aligners I can get moulds for any refinements that I want on my top arch and then switch to <span id="SPELLING_ERROR_4" class="blsp-spelling-error">retainment</span> mode for the top arches until they arrive. I will continue to wear each of the remaining 17 sets of lower aligners as usual but only my last set of top aligners  at night. I had to double check this with him twice, but apparently that is the case; nighttime wear only will be fine.  He reiterated that I have three attempts at refinement included in his price so I am happy to use one up in fiddling with the top arch whilst the bottom completes it&#8217;s first run. I&#8217;m not sure exactly what I&#8217;ll want to change yet, but I already have a few ideas of things that I&#8217;d like to tweak, so no doubt I&#8217;ll have a long, perfectionist, type A list by the time we make the new moulds in a few weeks!</div>
<div>My orthodontist has said that I can also get my new crown to replace the ugly old one that is really showing it&#8217;s age lately and that we&#8217;ll do refinements around it. Initially, this was definitely what I wanted, however, one of my two front teeth is not quite in the position that I want it to be yet, and unless it moves fairly significantly in the next two aligners I think it will need to be worked on during refinements. As the new crown will be right next to it, I have the feeling that I might be better off leaving it until the end of the whole process and getting the crown made then to perfectly fit the shape of the final gap. I&#8217;ll make a decision on this in the next few weeks, but please,let me know what you think. Bear in mind that a new crown costs $1500. Gulp. I really don&#8217;t want to have to do it twice.</div>

<div>My orthodontist is a lovely guy and I usually enjoy the banter that his visits entail but I have to say that I enjoyed this one a lot less than previous ones. Not the fault of my orthodontist, rather that I had an attack of heart palpitations in his office. The last few months I have been having these a lot, when I bend down and stand up too suddenly, when I twist into certain positions, and also I now know, when I get tipped upside down, head <span id="SPELLING_ERROR_5" class="blsp-spelling-corrected">above</span> my feet, in a dentists chair.</div>
<div>My heart, for some strange, electrical reason goes into a crazy irregular pattern for 30 seconds -2 minutes. I have seen two cardiologists, one of whom specialises in electrical problems, and had numerous tests and they have assured me that it is nothing serious, but still it is unnerving and <span id="SPELLING_ERROR_6" class="blsp-spelling-corrected">extremely</span> unpleasant. Luckily, my <span id="SPELLING_ERROR_7" class="blsp-spelling-error">orthodontist</span> has had some experience of this and he and his assistant were very calm about it and just kept my head above my feet for the rest of my</div>
<div>appointment, meaning that it quickly stopped, but it was a little scary nonetheless.  I plan to do all the rest of my appointments in a more upright position: I&#8217;m prepared to put up with a lot for my new teeth, but heart <span id="SPELLING_ERROR_8" class="blsp-spelling-corrected">arrhythmias</span> weren&#8217;t <span id="SPELLING_ERROR_9" class="blsp-spelling-corrected">something</span> I had planned for!</div>
<div>Set 15 has been going very smoothly, no pain, little discomfort and no other problems. There is a tiny gap between my tooth and the aligner along the bottom of one of my front teeth, but I showed my orthodontist and he was happy that it was normal so I think all is well. I must confess that I haven&#8217;t been wearing them the full 22 hours of late so whether this is my comeuppance only time will tell. I will try to be more disciplined over the next two sets. No really, I will.</div>
<div>Here are the hideous photos. Can I just point out in my defence that the water on them is from the things that hold your mouth back, whose name entirely eludes me. It&#8217;s <strong>not</strong> my saliva! Honestly.</div>

<p>I finally have photographic evidence that my teeth are really, really improving. I took my daughters to a function last week at which there was a professional photographer taking photos. Usually in such situations I just do my usual closed mouth, tight lipped, teeth concealing smile. However, this was a woman photographer and she was fierce! There was no way she was going to accept anything less than a full on toothy, Hollywood smile. So, in the interests of leaving there in one piece I went for it.</p>

<div>The amazing thing was that I saw the pictures on screen and my teeth look good. I mean really good, like straight, well proportioned teeth. <span id="SPELLING_ERROR_0" class="blsp-spelling-error">Ok</span>, they aren&#8217;t yet Hollywood perfect, but compared to how they were before I started <span id="SPELLING_ERROR_1" class="blsp-spelling-error">Invisalign</span>, they are fantastic. I think what has changed is that my upper arches are starting to widen and fill out my mouth. One of the reasons (and not the only one, believe me!) that my teeth used to look so terrible is that I have a Julia Roberts sized mouth, but with a toddler sized set of teeth. My arches simply weren&#8217;t wide enough to fill my smile. Thankfully that seems to be changing.</div>
<div>I have spent a lot of time looking at close up <a rel="nofollow" target="_blank" rel="nofollow" href="http://www.myinvisalignblog.com/2009/05/comparison-pictures-set-1-10-of.html">comparison pictures</a> of my teeth, analysing them for tiny changes without realising that, actually, it is from a distance that the improvements are most noticeable. Even my husband told me this week &#8220;your arches definitely look wider. &#8221; Of course, being a woman I had to ask him, &#8220;Does that look better?&#8221; to which I got the response &#8220;<span id="SPELLING_ERROR_2" class="blsp-spelling-error">ummm</span>, yes?&#8221;</div>
<div>The thing is whether he thinks so or not, I think they look better. It is a little strange though, to see this new look evolving. I have always seen myself as someone with <a rel="nofollow" target="_blank" rel="nofollow" href="http://www.myinvisalignblog.com/2008/11/photos-of-my-teeth-pre-invisalign.html">bad teeth</a> and to suddenly find myself needing to reevaluate that self image  is exciting. Yes, I know it&#8217;s pure vanity, and vacuous, and all those other terrible things, but hey, let&#8217;s be honest, this whole $6300 dollar escapade is really just an expensive beauty treatment. I might be telling people that I&#8217;m doing it because my bite alignment might cause me jaw pain in later life, but the reality is&#8230; well, I made that story up to hide my embarrassment over how vain I am!</div>
<div>I am including below a comparison of my set 1 and set 11 aligners. There is  definite evidence now of the widening of my upper arches. I can&#8217;t for the life of me see a lot of difference on the lower arches other than that the gap is beginning to open to allow my wonky tooth to move in. I&#8217;ll let you be the judges though.</div>

<p>My orthodontist seems like a nice guy. He is certainly experienced with Invisalign, is funny, reassuring and seems to know what he is doing. Generally he fills me with confidence. I must confess I wasn&#8217;t wild about his lectures to his postgrad students on my teeth being called <a rel="nofollow" target="_blank" rel="nofollow" href="http://www.myinvisalignblog.com/2009/03/6th-set-of-invisalign-braces-silly.html">&#8220;A silly thing to try and do with Invisalign&#8221;</a> but I took it in fairly good heart. However, when at my latest visit he told me that if what he is trying to do to my teeth with Invisalign works, he will write about it for the medical journals, I really started to get nervous&#8230;</p>
<div>Funnily enough, I don&#8217;t like the sound of that. Not that I mind being in the medical journals- I have been in hundreds of magazines in my prior career as a model and presumably I wouldn&#8217;t be identified, so that isn&#8217;t a problem. Rather it is the feeling that I am some kind of orthodontic guinea pig that is scaring me.</div>
<div>Now I should be fair here. My orthodontist was extremely straightforward with me and told me from the outset that the tooth that I have at the bottom of my mouth would be very hard to move. <a href="http://myinvisalignblog.com/2009/01/invisalign-clincheck-videos.html">See my clincheck here</a>. He also warned me before treatment started that there was a good chance that it won&#8217;t move at all, and that even having created a space for it, it may just not move up into the place we make for it. If it does, I will need a fixed brace on that tooth and the ones next to it to get it to move. I know all that, and I accept it fully. If the tooth in question fails to move I will have that tooth extracted and either close the gap again or get some kind of implant tooth or similar.</div>
<div>I think what maybe I didn&#8217;t appreciate, however,  was that it hadn&#8217;t really been tried before, or that if it worked it would be such a new technique for Invisalign  it would <span style="font-weight: bold;">make the medical journals</span>! I thought it was more of a &#8220;this could be a bit tricky&#8221; scenario, rather than a &#8220;this might be a world first&#8221; Still, I am going to remain positive, nonetheless. Repeat after me&#8230;.I believe that my teeth can move, I believe&#8230;</div>
<div>Other than scaring me half out of my wits the appointment went well however. He did a tiny bit of IPR even though it wasn&#8217;t on my schedule, just because he felt that two of my teeth were a little close together and might have difficulty moving past each other.  I haven&#8217;t had IPR before and it wasn&#8217;t the most pleasant of experiences but it wasn&#8217;t <span style="font-style: italic;">awful</span>. He simply took some type of  metal file and slid it into the gap between my teeth. He then moved it backwards and forwards between my teeth in a sawing action. There was a grinding, grating sensation, and it very definitely felt like he was shaving the sides of my teeth off, but there was absolutely no pain whatsoever. I spent the whole time a little petrified that he would somehow miss and file my gum, but that didn&#8217;t happen.</div>
<div>Other than that everything is on track. I picked up sets 9-11 and will put set 9 in tonight. The great news is that comparing sets 1-11 I can definitely see some differences in the aligners now, so I shall post some <a rel="nofollow" target="_blank" rel="nofollow" href="http://www.myinvisalignblog.com/2009/05/comparison-pictures-set-1-10-of.html">pictures</a> of that over the next couple of days.</div>

<p>When I originally went looking for an orthodontist (read about the whole sorry saga <a rel="nofollow" target="_blank" rel="nofollow" href="http://www.myinvisalignblog.com/2008/11/search-for-orthodontist.html">here</a>) one of my prerequisites was that my dentist should be an Invisalign Platinum Provider (Invisalign Premier provider in the US). At the time I was looking this seemed to be the blue chip of Invisalign orthodontics, a sign that the Doctor in question was one of the most experienced orthodontists or dentists in using the Invisalign system.</p>
<div>Then the other day whilst at my orthodontists I noticed a new plaque on his wall. This one congratulated him on being a Platinum <span style="font-style: italic;"><span style="font-weight: bold;">Elite</span> </span>Invisalign<span style="font-style: italic;"> </span>provider. I was intrigued. Was this just another name for platinum providers, or had the fine people at Invisalign created a whole new tier for our goodly orthodontists to aspire to?</div>
<div>Well a quick search on the Invisalign website has enlightened me. Apparently a platinum provider is the name given for &#8220;Doctors who have a high level of Invisalign experience based on the number of patients treated&#8221; whilst Platinum Elite providers (Invisalign premier provider elite) are &#8220;Doctors who have the <span style="font-weight: bold;">highest</span> level of Invisalign experience based on the number of patients treated&#8221;</div>
<div>The receptionists at my orthodontists office also told me that Elite status is reassesssed regularly and only those orthodontists that complete enough cases on an ongoing basis retain the Elite title.</div>
<div><span style="font-style: italic;">As a post script I have just found out that there are silver, gold, platinum and platinum elite providers in the Uk and some other countries!</span></div>

<div>I&#8217;m beginning to feel that my Invisalign braces and I are a bit like an old married couple. We muddle along through life quite happily with each other, they irritate me every now and then, but generally I  just ignore them and they don&#8217;t bother me too much. The only problem is that I probably neglect them a little these days, and don&#8217;t shower them with attention as I did in those first few heady weeks.</div>
<div>At the beginning of this experience I used to read on the various <span id="SPELLING_ERROR_2" class="blsp-spelling-error">Invisalign</span> forums of people who didn&#8217;t brush their teeth after meals, sometimes left their braces out for a whole evening and generally didn&#8217;t follow their orthodontists advice. I would post my self righteous responses, recommending they wear them more, brush and floss more, generally be more diligent in their care of their teeth. Then one day last week I realised that I was now one of those people. How the mighty are fallen!</div>
<div>The analogy of the old relationship is true. I didn&#8217;t mean to, and I&#8217;m not even sure exactly when it started but I can feel myself sliding into bad habits. So this week, starting tomorrow (when else!) will be braces boot camp. No more lingering over long coffees. No more taking them out for breakfast and not putting them back in before I leave the house an hour later for the school run. I need to get this relationship back on track before it goes rotten. Before my teeth are filled with cavities and my aligners are filled with air where my teeth haven&#8217;t moved where they were supposed to.</div>
<div>Despite my lax attitude over the last week or so the impact on my teeth as far as I can tell has been minimal. I did find the first time I went to insert the bottom aligner that it was almost impossible- in fact I took it out to check the numbering because it was so difficult. Once in however, they felt tight for a few minutes but then were <span id="SPELLING_ERROR_3" class="blsp-spelling-corrected">completely</span> comfortable. They weren&#8217;t even difficult to remove (although they were a little difficult to insert for the whole first 2 days) Now though, just about a week in, they are as <span id="SPELLING_ERROR_4" class="blsp-spelling-corrected">comfortable</span> as an old, much loved pair of trainers.</div>
<div>I have posted  <a href="http://myinvisalignblog.com/2009/03/i-spent-most-of-my-last-post-bemoaning.html" target="_blank">pictures</a> above so that you can see the progress from my first to seventh set of aligners. The visible changes are still very minor, although I can probably see them more easily in my mouth. The relationship between my top front teeth and my incisors next to them shows the most change. Also, although you can&#8217;t see it in the pictures a gap is starting to open up that my crazy <a href="http://myinvisalignblog.com/2009/01/invisalign-clincheck-videos.html">bottom tooth</a> can move into.</div>
<div>The other great event this week has been the much longed for arrival of my mother in law. All I can say is that she has done an admirable job of restraining herself- not a word has been said about my braces, and it must have been so very tempting for her! My father in law, however, took me aside 2 minutes after his arrival and asked me whether I had &#8220;new teeth&#8221; So much for the invisibility that we all pay so much for! He was probably the only person I know that would be forthright enough to actually ask, so I suppose they must really be fairly visible, at least to people that already know you and how your teeth look.</div>
<div>Still, even if people I know can tell the difference I still think the <span id="SPELLING_ERROR_6" class="blsp-spelling-error">Invisalign</span> are worth it. I would rather have less visible braces now and my teeth slightly less perfect later, than have metal braces now and have perfect teeth. Isn&#8217;t vanity a funny old thing?</div>

<p>I chose my Orthodontist after a long and arduous <a rel="nofollow" target="_blank" rel="nofollow" href="http://www.myinvisalignblog.com/2008/11/search-for-orthodontist.html">search</a> for the right <span id="SPELLING_ERROR_0" class="blsp-spelling-error">Invisalign</span> doctor. One of the factors that helped me to choose him was that he is a lecturer in orthodontics at a prestigious Australian University. It never crossed my mind though that I might become the basis for one of his lectures.</p>
<div>Apparently though, that is the case. I don&#8217;t mind that at all- as far as I am concerned if my teeth can help other orthodontists to improve their <span id="SPELLING_ERROR_1" class="blsp-spelling-error">Invisalign</span> technique I will be delighted. It is more the title of the lecture that bothers me. My orthodontist cheerily informed me yesterday that my teeth are the basis for a lecture he is giving next week, entitled&#8230;&#8221;Silly things to try and do with <span id="SPELLING_ERROR_2" class="blsp-spelling-error">Invisalign</span> braces&#8221; That makes me feel so, so much more confident! My only consolation is that he might need to provide an update to the students in the future, so perhaps this will encourage him to do his best work&#8230; How much do I wish I could be a fly on the wall and hear what he has to say!</div>
<div>Other than that, my 6 weekly visit was short and uneventful. I asked my <a rel="nofollow" target="_blank" rel="nofollow" href="http://www.myinvisalignblog.com/2009/02/diet-coke-flouride-and-invisalign.html">query about <span id="SPELLING_ERROR_3" class="blsp-spelling-error">flouride</span></a> and he said that as we have <span id="SPELLING_ERROR_4" class="blsp-spelling-error">flouridated</span> water here now flouride probably isn&#8217;t necessary for most patients. However, in patients with large amounts of fillings (i.e. me) it can do no harm. He actually suggested a weekly <span id="SPELLING_ERROR_5" class="blsp-spelling-error">flouride</span> rinse. His only stipulation was that it should be a <span id="SPELLING_ERROR_6" class="blsp-spelling-error">ph</span> neutral one.</div>
<div>I picked up three more sets- sets 6, 7 and 8, and immediately put set 6 in. They are <span style="font-style: italic;">tight. </span>For the first time since set 1 at the beginning of this process I actually felt I really had to take a painkiller. I should probably explain something here. I have <span id="SPELLING_ERROR_7" class="blsp-spelling-error">crohns</span> disease which is an inflammatory stomach disorder. As a consequence I have a lot of stomach ulcers and try and avoid painkillers as much as possible. When I do take them I try and stick to paracetamol. So I don&#8217;t take them lightly, taking one maybe 2 or 3 times a year (as opposed to 2 or 3 times a week like my husband!). Lots of people, I know, always pop a painkiller when they put in a new <span id="SPELLING_ERROR_8" class="blsp-spelling-error">Invisalign</span> tray in, but generally I haven&#8217;t found it necessary. This time though the pain became so much that I had the sense that it was going to give me a migraine, so I decided to preempt that. It seemed to work because by next morning they felt a lot better.</div>
<div>I have had the usual difficulties of removing them with the attachments, and have noticed that there is a tiny crack in the side of one of my molars on the bottom arch. I am ignoring this unless it gets worse as it doesn&#8217;t seem to be <span id="SPELLING_ERROR_9" class="blsp-spelling-corrected">compromising</span> the strength of the tray. I know that probably I should call my orthodontist, but I know that he will only have two options- to order a new tray, which I don&#8217;t want to wait for, or to push me forward a set, which I don&#8217;t want to do. So, I am going to try and wear these as long as I can of the two weeks. If they get weaker or snap I will, of course, call him.</div>
<div>I live in Australia, having emigrated here four years ago. Consequently all my family are still in the <span id="SPELLING_ERROR_10" class="blsp-spelling-corrected">UK</span>, and haven&#8217;t yet seen my <span id="SPELLING_ERROR_11" class="blsp-spelling-error">Invisalign</span>. They are all arriving next week, and I am particularly looking forward to the comments from my Mother In Law who utterly despises me and will no doubt have some interesting things to say about my braces-I&#8217;ll keep you posted!</div>
